L3HFMC accepts most major health insurance plans, including Aetna, Blue Cross Blue Shield, Cigna, Health First, Humana/Tricare, Medicare part B, and United Healthcare (applicable copays/cost share will apply). Please contact the center at 321-726-1600 to verify if your particular plan is covered.

Retirees who were enrolled in a L3Harris plan as of 12/31/14 are eligible to use L3HFMC, but should check with their current insurance carrier to determine whether L3HFMC is in-network or out-of-network. You may also contact L3HFMC to help estimate any costs for services rendered at the facility.

Formulary Brand – You pay 25% of cost ($35 min; $100 max) after deductible met
Non-Formulary Brand – You pay 50% of cost ($50min; $100 max) after deductible met
