Appointments

Call (321) 726-1600 to schedule an appointment during our regular exam hours, 8:30am – 5:30pm. We also offer extended-hours (early morning and late afternoon) appointment times to meet the needs of our patients, 7:00am – 6:00pm.

Urgent Care

L3HFMC accepts minor emergencies (e.g. sprains, strains, lacerations, nausea, fever, earaches and minor injuries) on a walk-in basis during office hours with the first available physician.

Important information about labs: As a courtesy, we will draw labs if one of our physicians orders them. Quest will bill your insurance directly so co-pay/co-insurance may apply, depending on which insurance plan you have and their coverage.

After-Hours Care

A staff physician is on-call to handle urgent situations outside normal business hours for L3HFMC patients by calling (321) 726-1600. For non-urgent issues, such as, prescription refills or lab/imaging result inquiries, please log on to the L3HFMC patient portal to leave a message that will be answered the next business day.

Emergencies: If you are having a serious emergency, such as chest pain, shortness of breath, bleeding, etc., call 911 or go directly to the nearest emergency room! If you need to speak to someone at L3HFMC on an urgent basis, call (321) 726-1600. Do not use e-mail to contact the L3HFMC for potentially emergency-type problems.

You may use L3HFMC’s Patient Portal to contact your PCP doctor for follow-up questions, status reports and/or updates on your condition. For new or complicated issues, please schedule an appointment to see your doctor. Your illness will not be diagnosed or treated via e-mail.

    Generic – You pay full cost up to $10 after deductible met
    Formulary Brand – You pay 25% of cost ($55 min; $100 max) after deductible met
    Non-Formulary Brand – You pay 50% of cost ($50 min; $100 max) after deductible met
  • Maintenance Drugs (90-day supply):
    Generic – You pay full cost up to $25 after deductible met
    Formulary Brand – You pay 20% of cost ($87.50 min; $250 max) after deductible met
    Non-Formulary Brand – You pay 50% of cost ($125 min; $250 max) after deductible met

*Pharmacy benefits – other than certain preventative drugs – are not payable under the THP or HDHP until the deductible is met.

HIPAA Information

On April 14, 2003, the Health Information Portability and Accountability Act (HIPAA) went into effect for all U.S. medical facilities. HIPAA is designed to assure the confidentiality, integrity and security of patient health information. The L3Harris Family Medical Center (L3HFMC) is compliant with Florida’s privacy law and has implemented all necessary procedures to be in compliance with HIPAA. We will post our Notice of Privacy in all waiting rooms and on the L3HFMC website.

In order to continue to protect patient privacy we ask all patients to respect the privacy of other patients. This may include being asked to have a seat in the waiting area until we can accommodate you at the registration desk, pharmacy window and/or patient care area. Due to the nature of the conversations that could be overheard in the patient care area, any friend or family member that accompanies a patient must stay in the exam room with the patient. If the patient objects to having anyone in the exam room while the doctor examines them, they will be asked to stay in the waiting room.

We will also need to obtain your authorization to leave appointment confirmations on your work and/or home phone and/or to send an email that may contain protected health information. Communication release forms are linked below.

HIPAA requires healthcare providers to obtain written authorizations from the patient to release any protected health information that is not related to your treatment, payment, healthcare operations or required by law. For example, we will need to have a written authorization to release or fax immunization records to the public schools.
